OPERATION LOOKOUT® has designed the "EZ Form Registration" to simplify the registration process for you.  To begin downloading please select the appropriate EZ Form Registration Packet according to the classification of your case.

• Family Abduction (110kb) - Use this packet if a parent, grandparent, aunt/uncle, brother/sister, etc. abducted the child(ren).
• Non-Family Abduction (110kb) - Use this packet if your case is stranger, unexplained, or a known person who is not related to you through blood or marriage.
• Runaway (110kb)

Note:  You will be assigned a case number and activation of your case will begin when all necessary forms/documentation is received. Please get all the information to us as quickly as possible so we can activate your case without delay.
