Registering your missing child(ren) is as simple as 1-2-3. The fastest way to get your child(ren) registered with OPERATION LOOKOUT® is to start by pre-registering, expediting your documents and photos, then sending the originals to us. Your case will be activated and assigned a case number when all of the original forms and documentation set forth in the registration packet are received in our office.
Pre-Register your child(ren) using the link below. You will be asked some general information about yourself and your child(ren). Please fill in the information as complete as possible. The information you provide will help to prepare your Case Manager. Once you have completed the form and sent your information online, you will be directed to the download location of the "EZ Form Registration Packet".
Download the registration packet for your case type. Print, complete, and notarize the necessary forms included in "EZ Form Registration Packet" (you may request a packet be mailed or emailed to you by
contacting us). Assemble required documentation as set forth in the registration packet.
Send your documents electronically or by mail. -- **If you intend to return your required forms and documentation to us by electronic means, an "Ink Seal" notary is required.
