Include only the information you are comfortable with giving out, although first name, last name, and email address is required. Any information you provide will be used for the purpose of Operation Lookout's mission to search for and recover missing children.

Remarks:

Here's the place to let us know what you think about our website, what we do, anything in general. Please don't send any information to us regarding a sighting or to report a missing child from this area. If you do have information about a child or need to report a missing child, call Operation Lookout at 1-800-LOOKOUT ext. 1234, available 24 hours a day, 7 days a week. If you choose to print out the form to mail or fax it to us, please limit your comments & suggestions to the viewable area of the box. Anything you enter beyond it, we won't receive.

Missing Child "E"Lert™:

"E"Lert™ stands for Electronic Alert Poster Partner. The Missing Child "E"Lert™ is our quickest way to get a child's information to the public. We ask that you forward the "E"Lert™ email you receive to as many people as you know to help us spread the word that the child(ren) is missing. This "E"Lert™ email will contain at least 1 link to a printable poster of the missing child which is ready for download and circulation. We've also included in the form an area where you can refer 3 people who you think would like to receive "E"Lert™ and we will include them in the next emailing.

Note: Adobe Acrobat Reader is required to view and print the poster. If you don't already have it, it's available for almost every operating system. Get it from Adobe free.

Donations:

Please download and include our Donation Form (124k) when sending check or money order via USPS. You may use the form to fax/mail credit/debit card donations if you do not feel safe sending your credit card information over a secure server on the internet.
